RN: Case Manager

Title - RN: Case Manager
Location - Brunswick, GA
Shift - Monday- Friday with on call- 8 am to 5 pm, On Call would be once every three weeks.
License Requirements: GA RN License, willing to accept compact license
Productivity Requirement: 30 units, Hours are not guaranteed.
Mileage reimbursement: 0.43 cents per mile
Counties covered: Glynn, Camden, Brantley, and Charlton
Preferred Skills: wound care, iv care, HCHB experience, and home health experience
Location of clinic: Brunswick
Primary Responsibilities: Perform comprehensive assessment of high-risk members. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member.
